Preparing for your appointment/visit
Please arrive at the office with all three forms filled out and signed. You will need to bring your driver’s license/photo I.D. along with your insurance cards. If you do not have your insurance card, we will need the subscriber’s name and date of birth, contract/I.D. number, and the phone number of your insurance company. Please contact your former dentist to have copies of your previous x-rays sent to our office. They can be mailed or emailed to our office.
